I agree to be invoiced for Arts Integration Certification at this time. I understand the information on this form will be used to issue me an invoice. This invoice must be paid via ACH or Credit Card (no paper checks are accepted) within 30 days or it will be voided. Access to the System will be provided upon receipt of payment.
